How to make a Request for SLT Assistance

  • Anyone (a professional or a parent/carer) can make a request for SLT Assistance.

 

  • Please note that permission from the parent/carer is required when making a request for SLT Assistance.

 

  • Before completing the Request for Assistance form please consider the following:
  • Use the information you have gathered as a basis to discuss this with the parents/carers. Their permission must be obtained before completing the Request for Assistance form. You can find the correct form for your area here.

 

  • Please include as much relevant information as you can about the child’s specific difficulties such as their listening and attention skills, understanding, use of language; including your reason for making the Request for Assistance. Also include up to date addresses and telephone numbers, placement details and details of other relevant professionals involved.

First Appointment

The first assessment usually takes place at the local clinic. However, in some circumstances, the assessment may happen elsewhere, e.g. at a Nursery or Early Years Centre.

Parents/carers are directly involved in assessment and any planned intervention. Their attendance at the initial appointment is vital in order to gather information and complete a case history.

After this initial assessment, the therapist will discuss the next steps with the parents/carers e.g. further assessment, intervention, advice, and support or no input necessary.

The individual making the Request for Assistance will be informed of the outcome.
